Hey Callie, big hug for starters, you’re not alone here!
You know there’s only one star like you in this whole universe? Seriously, think about that… All the potential destinations only you could reach… Take it one day at a time to begin with, neither yesterday nor tomorrow define you in any way, shape or form! You’re perfect and worthy just the way you are - it’s your lifestyle which needs to change to accommodate that…
Reflect upon and ask yourself when, where, why and how your addictions/problems might have started? They can often just be just the tip of an iceberg and it takes some soul-searching to lay out exactly where you’re at in life, why and how to move on from here… What is life all about for you? What can you envision your ideal, free, conscious and happy future self saying to you now? What put/puts a smile on your face or made/makes you feel alive?
Have you ever considered a psychologist/therapy/recovery group? It could just do you a WORLD of good if you’re open enough to it 
What comprises the big picture in life is the small things, one day at a time doesn’t go without saying. What you do today will largely shape your tomorrow. Note to yourself as many answers to: who, when, where, why, what and how as relevant to your life given your circumstances? What helps a LOT is the simple but so important things like hygiene, tidiness, healthy sleep, hydration, a healthy diet, exercise, fresh air etc, here some suggestions 
Learn or play an instrument? Learn a new language (maybe on duolingo)? Join a club/organisation/group of even remote potential interest? Could be dancing, self defense, a volunteer group helping others, a cause? Exercise/calisthenics/running? Walking/hiking? Cycling? Swimming as you’ve already mentioned!!
Go out into fresh aired, beautiful nature, if not the real deal even parks/botanic gardens etc can be nice mind revivers! What activities do or might you find fun or productive? Same goes for groups of likeminded people! It’s not all just about fun, progress and achievement is a huge booster! Physical and mental activity and progress is more rewarding than anything, hence why going for a run, cycling, swimming or exercising make you feel better… Same goes for psychological growth, whatever direction you take that in! How about just composing a big list of potential hobbies, goals, ideas, plans, interests etc? How can you realistically get from here to there? The more you try, the more you care and vice versa - you’re still in the early phase so don’t be too hard on yourself! Growing takes time but is 100% worth it… Note your failures, successes and thoughts just to get to know yourself. Please don’t hesitate to open up to and work this through with others who love and care for you like you deserve
And don’t chase a light which is ALREADY right there within you and ALWAYS was!
Stay strong, keep your head up, be proud of being here and getting this far in the first place Callie, feel your pulse and breath for a while just to remember that you’re actually alive, meditate, be yourself and focus on the present!
